Junior Camps Letter of Permission

It’s that time of year when players start receiving invites to Junior camps. Players that live in Calgary should complete the form on the Hockey Calgary website at https://www.hockeycalgary.ca/forms/letter-of-permission for each camp you attend.

A lot of Junior teams will direct you to complete the Hockey Alberta form at https://www.hockeyalberta.ca/junior-senior-letter-permission-tryout/. If you use the HA form please note the following:

Resident MHA/Club Team – Enter Hockey Calgary and your team. Ex Hockey Calgary U15 AAA Bisons.

MHA/ Club Team President – Enter Kevin Kobelka

MHA/Club Team President email – enter Pam.Douglas@hockeycalgary.com

The CBHA does not need to approve or be sent the forms for Junior camps. If you have any questions please email me at lori.mcd@outlook.com
